Appointments for New Patients

Thanks for choosing Wasatch Vision Center for your eye care needs. Once you have made an appointment with us, you will receive a link via email or text to complete your paper work before you come in for your appointment. If you are unable to complete it before your appointment, we will have an iPad at the front desk for you to finish your check in.

New patients will fill out a new patient Medical History Questionnaire, Insurance Verification Form, and read through our HIPAA Privacy information.

If it has been 3 or more years since your last appointment with us you may be asked to complete some paper work. This is so we can keep your records accurate and up to date.

Cancellation Policy

As available appointment times are booking out further all the time, we are striving to accommodate everyone's needs. In an effort to provide care for all our patients in a timely manner, we are implementing a cancellation policy. No-shows, late arrivals, and last minute cancellations inconvenience not only our providers, but our other patients as well.
In order to be respectful of the doctors and your fellow patients, please call Wasatch Vision Center as soon as you know you will not be able to make your appointment.
If cancellation is necessary, we require that you call at least 24 hours in advance. Appointments are in high demand, and your advanced notice will allow us to fill all available appointment times.
If you need to cancel your appointment, please call us at 435-654-1863 between the hours of 9:00 AM – 5:00PM.
Late cancellations (less than 24 hour notice) and no-shows will be charged a missed appointment fee.
